Mmm... I wouldn't translate "bad", because it would sound a bit weird, in italian.
"Perdona il mio italiano" [ Informal, but a bit too formal :P ]
"Scusa per il mio italiano" [ informal ]
"Chiedo scusa per il mio italiano" [ A bit formal ]

No. But you could say "Scusa per il mio brutto/pessimo italiano"
PS: Why have you put "male" after the comma?

to emphasis that my italian is bad but adding the word brutto is just as good thanks... :)
