"Prenditi/Abbi cura di te (stesso)"
or
"Bada a te (stesso)" [ A bit uncommon ]

"Prendetevi/Abbiate cura di voi (stessi)"
or
"Badate a voi (stessi)" [ A bit uncommon ]

As for "Prendersi cura di te/voi", it can be used only as a subordinate clause, because the tense is infinitive.

i.e.:

"Gli dirò di prendersi cura di te/voi"
"I'll tell him/them to take care of you"

So the best way to say, "take care of yourself" would be "prenditi cura di te" correct?

Both "Prenditi/Prendetevi cura di te/voi" and "Abbi/Abbiate cura di te/voi" are OK. ;)
There's also a more colloquial and common expression:

"Riguardati" [ Singular ]
"Riguardatevi" [ Plural ]
