There isn't an exact translation for that expression, but I'd say "Ho lo stomaco sottosopra" [ "I have an upset stomach" ].
"What can it be?" = "Cosa può essere?"
As for "What is?", if you're asking someone for the exact symptoms, you could say "Cos'hai (di preciso)?" or "Cosa ti senti (di preciso)?"
You can use "Esattamente", instead of "Di preciso".

what about saying "I have a stomach ache" or a "bad stomach"

It's the same expression you would use for headache. "Ho mal di testo"

"Ho mal di stomaco" is ok, but is "Stomach ache" the same as "Bad stomach"? That's why I haven't told you that translation.
BTW, it's "Testa", not "Testo", which means "Text".

Also, there are different kinds of stomach aches but if you are able to communicate that your stomach hurts I think then the conclusion can be drawn that your stomach is bad, for me the phrases are interchangeable. grazie... :)

Well, that's not exactly the same.
If my stomach is upset, it doesn't mean it's necessarily hurting.
BTW, another kind of stomach ache is "Acidità/bruciore di stomaco" which translates as "Sour stomach".
PS: "Il mio stomaco è male" doesn't make sense. It sounds like "My stomach is evil". :D

We'll have to disagree on that because if a person's stomach is upset it doesn't feel good. There may be some subtle pain or discomfort but I don't know anyone that has an upset stomach and not complain about it. In any event, I do understand.. Thanks again Sal.. :))
