The difference between grandchildren and niece/nephew

If il nipote/la nipote means grandson/daughter and il nipote/la nipote means niece/nephew, how to tell the difference between them when using them in a sentence. How do you know when a sentence is referring to a grandson/daughter and/or niece/nephew?

It all depends on the context. ;)
We often ask people to be clearer, when talking about "nipoti". :D

He/She would say "Mio nipote è in casa". ;)
If you don't know if he/she's referring to a grandson or a nephew, you could ask "Lei è suo/a nonno/a o suo/a zio/a?" [ Are you his grandfather/grandmother or his uncle/aunt ].
I used "Lei", because we often use the formal form with the elderly, unless we're speaking to our own grandparents ]. ;)

But if someone says, my grandson is in the house. Would they say, "Mio nipote è nella casa." How would I know if this is talking about the grandson or the nephew?
